Homecare Services
To be eligible for Homecare services, persons must be:
At least sixty (60) years old
Functionally disabled and in need of assistance with their activities of daily living to prevent placement in a nursing home
Case Management Services
An interview with a nurse is held to determine a clients situation, what assistance is needed and the best way to provide it.
Case Managers then develop plans of care to provide and coordinate services for the older individual to assure that their needs are being met.
| CHORE | Heavy housecleaning and minor home repair services to maintain the person in his/her own home. |
| ESCORT | Personal assistance with transportation to and/or from their medical and other essential services. |
| HOME DELIVERED MEALS | Meals delivered to in-home, frail older persons who would otherwise be unable to obtain a nutritious meal. |
| HOME MANAGEMENT | Household activities including housekeeping, meal preparation, laundry, and shopping which are necessary to maintain an individual in his/her home. |
| HOME REPAIR | Minor home adaptations including additions or modifications to the home environment to enable the elderly to maintain independent living and safety.

| RESPITE | Personal assistance, supervision, or cues for persons with the inability to perform one or more of the following daily living activities: eating, dressing, bathing, toileting, transferring in and out of bed/chair or walking. |
The Buffalo Trace Area Development District provides services on a non-discriminatory basis.
